The 2nd edition of Gloucestershire’s Transition into Adulthood Information Pack is now ready and can be downloaded here . Copies of the pack will be sent out to all schools and given to any disabled young person who has a statement of special educational needs. The pack contains useful tools and guidance to support young people through the transition process, as well as useful information and contacts.
