Health Action Plans – Health Action Plans (HAPs) are aimed at young people with learning disabilities, or with profound multiple learning disabilities. GUiDE & PALS - GUiDE is the health, social care and disability information service for Gloucestershire. They can help with enquiries about health conditions, accessing services and support groups, funding (through Funderfinder, a search of charities), benefits, advice and support for anyone using NHS services in Gloucestershire. PALS (Patient, Advice and Liaison Service) can help you if you have a concern about your NHS Service. PALS offer individual support to anyone using Community, Mental Health and Learning Disability NHS Services in Gloucestershire. They can act on your behalf to resolve difficulties as quickly as possible and can guide you towards other services too.
